[Massplanners] Roadway Layout Plans for Town Acceptance

Hello MassPlanners,
Athol recently had its first approved and completed subdivision since before the Great Recession. That developer is now requesting the Town accept the subdivision roadway, which was built to the standards of the Planning Board's Subdivision Rules and Regulations.
The developer's surveyor provided us with a set of mylar plans for the road acceptance plans with no signature block. I seem to recall from the past, such layout plans will need to be ultimately signed by the Planning Board and Select Board, if the road is accepted by Town Meeting Voters. Though not 100% certain if its both Boards.
Does anyone have a list of requirements what should be on the roadway layout plan for accepting such subdivision roads they could share? And if they recall it would include an area for Board(s) signature?
