[Massplanners] Paving checklist/form

To clarify, we are just making sure they are following the normal paving standards as specified in MassDOT that they should be following anyway in THEIR site plan that they PROPOSE. The checklist form will be reviewed by our peer review engineers Horlsey & Witten or Beals & Thomas as well as the DPW.

Hello fellow MA planners,

For the paving of roads in subdivisions, the planning department and the DPW are looking to create a checklist document for the developer to receive approval prior to starting paving. The checklist document will have things like slips of material and tonnage, equipment, sieve analysis, compaction report, etc. The goal is to make sure paving of subdivisions are of good quality and permission is received before starting.  Does anyone have an example form/checklist their municipality uses for paving requests/projects?
