[Massplanners] PARC Grant Usage Report

Fellow planners,

I was just asked to provide assistance to my Parks Commission with their application for a PARC grant. The Commission is submitting for construction funds to rehab the Town's largest active use park and would like to submit a Usage Report along with the application.

I have not had an occasion to create or submit such a report in the few PARC Grants I have worked on. Does anyone out in Planner Land have an example of a Usage Report from a previous grant they have submitted that you are willing to share with me?
